I'm curious what are the strategies for getting it because I'm gonna try myself. I played as Isaac, I went to the first item room and I didn't open… So i got the It's the Key achievement not long ago and thought i'd share my method, All i did was pick up one key as isaac and made sure that was the only thing i picked up, no bombs or pills or anything like that. I also only took my first item room item, which was ipecac. So that meant no devil or angel rooms either.
